Session 13 - Transitioning Patients With Wounds Across the Continuum of Care

The coordination of wound care throughout the continuum from acute to various post-acute settings requires true collaboration of interprofessional wound care team members. Seamless transitions from one setting to another are challenging.  Some wound patients experience an endless cycle of admissions and discharges. Providers are faced with an onslaught of economic and regulatory consequences when patients are needlessly readmitted. This session will focus on strategies nurses can utilized for better communication, accountability and documentation to facilitate transitions of wound care patients in post-acute settings.
