Pressure ulcers/injuries and incontinence often occur together.  Nurses must assess continence/incontinence status, related skin alterations, including pressure ulcers/injuries, and damage from exposure to urine or stool to determine the appropriate plan of care. This session will outline critical elements for skin/wound assessment and continence/incontinence and while providing practical tips.
